Effects of the treatment of varicose veins (Verizen)

What are the first signs of varicose veins?
Possible signs of venous disease include a feeling of heaviness and tightness in the legs as well as swelling in the ankles. Calf cramps at night can also indicate impaired vein function.
A distinction is made between
Primary or genuinen varicosis
is genetic without known pre-existing diseases of the deep leg vein system
Secondary varicosis
is the result of an occlusion in the deep venous system (e.g. post-thrombotic syndrome, accidents…)
According to current research findings, every 4th adult in our latitudes suffers from varicose veins of the lower extremities. The peak incidence is between the ages of 30 and 40 and more frequently affects women.
Other risk factors are
- Positive family history

- Pregnancy
- Overweight (body mass index >30)
- Standing and sitting for long periods
- Post-thrombotic conditions (i.e. overloading of the surface veins after deep vein thrombosis)
What complaints (symptoms) occur with varicose veins?
- Leg edema (leg swelling)
- Feeling of tension and heaviness in both legs
- Nocturnal calf cramps
- These in turn can lead to pain-related sleep disturbance at night
- Prolonged venous bleeding in the event of skin injuries (e.g. sport, everyday work...)
- Thrombophlebitis (inflammation of the superficial veins) due to thrombosis of the superficial veins
- Itching
- Deep vein thrombosis up to and including pulmonary embolism
- Dark discoloration of the skin on the lower legs
- Formation of venous leg ulcers (lower leg ulcers, "open leg")
How is venous disease (varicose veins, varices) diagnosed?
All venous diagnostics at our center are performed by clinical examination as well as by color duplex sonography and diaphanoscopy using infrared light.

These measures merely support the natural return flow function of the leg veins and prevent leg edema. However, this does not cure the varicose veins. For a definitive cure, the veins must be closed by laser or surgically removed.

The aim of treatment is to permanently normalize haemodynamics while observing the principles of aesthetics. The younger patients with varicose veins are, the earlier treatment should be recommended.

What is endovenous laser therapy (EVLT)?
EVLT is offered at our center as the method of first choice. An improvement in quality of life with a significant reduction in the recurrence rate and excellent cosmetic results make endovenous therapy the gold standard.
Local anesthetic is injected around the vein under ultrasound guidance. This produces a cooling, anaesthetic and haemostatic effect. The laser probe is inserted into the vein using a needle and the laser energy is emitted from the tip of the laser probe onto the inner vein wall at temperatures of up to 700 °C. The vein is then permanently closed and dissolved by the body. This closes the vein in the long term and dissolves it in the body.

What happens after the treatment?
We recommend wearing class II compression stockings for one week, day and night. Our patients can go home immediately after the treatment. A follow-up examination takes place a few days later.
After endovenous laser therapy, there may be slight swelling in the treated area. This reaction is individual, but all patients are advised to rest for 24 hours after the procedure and not to engage in sports or other physically strenuous activities.
Endovenous foam sclerotherapy or foam sclerotherapy
In this procedure, an agent that irritates the vascular wall, usually ethoxysclerol, is mixed with a high-percentage glucose solution and injected into the diseased vein. The sclerosing agent leads to an inflammatory reaction in the vein wall, causing it to stick together and eventually close. The treatment is performed on an outpatient basis and usually takes 15 to 30 minutes. A larger vein surface area can be treated due to the foaming of the sclerosing agent.
Open surgery
Traditional varicose vein surgery with crossectomy, vein stripping, phlebectomy and perforator ligation is increasingly being replaced by endovenous treatment methods and is only reserved for a few limited indications.
What complications can occur after the operation?
Complications after varicose vein surgery are very low. Injuries to the large vessels and deep vein thrombosis occur in less than 0.05% of cases. Hematomas (bruises) are slightly more common.
